“It Takes Two” to Reduce Unplanned Extubations
BCH-Oakland Neonatal Intensive Care Unit, Respiratory Care, Neonatology, and Patient Safety
Nursing, respiratory care, physician, and patient safety champions worked with NICU staff to reduce unplanned extubations (UPEs) by identifying the causes of UPE events, testing interventions, establishing best practices, improving data collection using an electronic reporting system, and increasing bedside communication during rounds. This continuous quality improvement work has resulted in a culture change within our NICU to reduce UPEs.
BCH-San Francisco CLABSI Prevention Committee (CPC)
The BCH-San Francisco CLABSI Prevention Committee (CPC) is a multidisciplinary, hospital-wide group focused on CLABSI prevention. CPC was launched in December 2017 to address a significant increase in CLABSI during the first half of FY18. The initial goal to reduce CLABSIs by 25% relative to the first half of FY18 was achieved, and FY19 ended with a 36% CLABSI rate reduction compared to FY18. Key drivers of the work include sharing process and outcome CLABSI data with the frontline, staff education, standardization of central line care, daily discussion of lines on rounds, staff education, and family engagement.
