The Advanced Heart Failure Comprehensive Care Center Team was nominated for changing heart failure care for local and regional communities. Over the last two years, the AHF CCC has had significant growth. The heart failure disease management population is now over 1,200 patients. The team has grown from a heart transplant program that typically performed 20 transplants per year, to now being a center who performs ~80 per year. The team has also worked tirelessly to ensure that quality outcomes are maintained. According to US News and World Report, UCSF is now rated "high Performing" in heart failure. The team has the largest volume of VA ECMO for cardiogenic shock in the Western U.S. (top 10 in the U.S.) and is the 8th largest HeartMate 3 LVAD implant in the Western U.S. in 2023 (26 programs).

The information is presented live (via zoom) by peers, for peers. Since 2017, over 300 APPs have completed the program.

UCSF Hepatitis C Project ECHO
TEAM PRIDE CATEGORY
The ECHO model is designed to increase access to specialty treatment in underserved and rural areas by providing front-line clinicians with the knowledge and support needed to manage patients with complex conditions including hepatitis C. UCSF HCV experts utilize videoconference technology to educate and mentor community providers on screening, diagnosis, treatment, and management of hepatitis C. Through a combination of didactic learning and community clinician case presentations, local providers gain the knowledge and confidence to successfully treat hepatitis C in their own clinics. To date, UCSF HCV Project ECHO has empowered over 134 clinicians across California to cure hepatitis C.
7 Long, Musculoskeletal Unit

7 Long and 7 East are the adult inpatient units providing care for adult orthopaedic and spine patients at Parnassus. Comprised of 37 beds and 100 nursing and PCA staff who are part of the multidisciplinary team which includes physical therapists and occupational therapists, pharmacists, case managers, nurse practitioners and physician assistants, as well as our physicians. The team demonstrates PRIDE values every day as we strive to provide the best possible care for our often complex patients, using a collaborative approach to identify ways to continuously improve care and support each other and our patients in an atmosphere of openness and respect for each other.
